Specialized Care Coordination & Aging Support

Every family has unique needs, and no two situations are exactly alike. I provide personalized care coordination and senior advocacy to help older adults and their families navigate changing needs, healthcare transitions, community resources, and decisions about aging with greater confidence, dignity, and peace of mind.

Hospital & Rehabilitation Transition Support

I help families navigate the transition from a hospital or rehabilitation facility to home by coordinating services, reviewing discharge recommendations, identifying needed support, and connecting families with resources that can help promote a safe and successful recovery.

Healthcare & Community Resource Coordination

Navigating healthcare and community resources can be overwhelming. I help families identify trusted services and resources, coordinate care, communicate with providers when appropriate, and connect older adults with support that best meets their individual needs.

Home Safety & Aging in Place

I help families identify opportunities to improve home safety, support independence, and plan for successful aging in place. When additional assistance or services are needed, I can help connect families with appropriate community resources and support.

Everyday Care & Support Services

Care needs often extend beyond traditional caregiving or a formal care plan. I provide practical, personalized support that helps older adults remain independent, connected, and engaged in everyday life while giving families the reassurance of knowing dependable help is available.

Companion Care

Meaningful companionship, conversation, activities, walks, social engagement, and a dependable presence help older adults stay connected and engaged. Companion care can also include accompaniment on social and community outings and participation in activities the client enjoys.

Respite Care

Dependable temporary support gives family caregivers time to rest, work, attend appointments, manage personal responsibilities, or simply take a needed break while their loved one receives attentive, compassionate care and companionship.

Transportation & Appointment

I provide transportation and personal accompaniment to medical and dental appointments, therapy, hair appointments, grocery shopping, pharmacy visits, errands, community activities, social outings, family visits, and other important destinations.

Activities of Daily Living (ADL) Support

I provide respectful assistance with everyday personal needs, including dressing, grooming, bathing and personal hygiene, toileting, mobility, transfers, eating assistance, and daily routines while encouraging as much independence as possible.

Light Cooking & Meal Support

I assist with simple meal preparation, light cooking, snacks, grocery lists, grocery shopping, meal setup, serving meals, and meal-related cleanup to help make everyday routines easier and more manageable.

Errands, Shopping & Community Outings

I assist with grocery and pharmacy visits, prescription pickup, household shopping, personal errands, restaurants, community activities, family gatherings, and social outings that help clients remain active, connected, and involved in the things they enjoy.

Planning & Family Support

As needs change, families may also need help organizing information, planning for future care, and connecting with trusted resources. I provide practical guidance and support to help families make informed decisions and feel better prepared for what comes next.

Medication Organization Support

I help families organize medication information, understand medication changes, prepare medication lists for appointments, and communicate medication-related concerns with healthcare providers. I do not administer medications or provide medical advice.

Long-Term Care Planning


I help families explore long-term care options, evaluate available resources, and develop a plan that supports their loved one’s current needs, preferences, and future care needs.

Family Education & Resource Connections

I connect families with trusted community resources, educational information, and support services that can make caregiving easier and help older adults remain safe, supported, and as independent as possible.
